Abstract

Background: Prescribing audits provide a basis for evaluating the appropriateness of antihyperlipidemic treatment in clinical practice. Objective: To describe prescribing patterns and assess documentation-based appropriateness at RSU Imelda Pekerja Indonesia, Medan. Methods: This descriptive study retrospectively reviewed medical records from March 2025 to February 2026. Data were collected in June 2026, and 51 patients were selected by purposive sampling. Patient, indication, drug, dose, and route appropriateness were assessed with reference to the 2021 Indonesian Society of Endocrinology (PERKENI) guidance. Data were summarized using frequencies, percentages, and exact 95% confidence intervals (CIs). Results: Women accounted for 32 patients (62.75%); the largest age group was 56–65 years (16; 31.37%). Simvastatin 20 mg, atorvastatin 20 mg, and fenofibrate 300 mg monotherapy were recorded in 26 (50.98%), 19 (37.25%), and four (7.84%) patients, respectively. Two patients (3.92%) received simvastatin 20 mg plus fenofibrate 100 mg. Statin monotherapy accounted for 88.24% of patients (95% CI, 76.13–95.56%). All 51 records were classified as appropriate for each criterion (100%; 95% CI, 93.02–100.00%). Conclusion: Statin monotherapy predominated, and no documented inappropriateness was identified within the five assessed criteria. These findings describe prescribing classifications, not comprehensive treatment safety or lipid-target attainment. Further evaluation should link prescribing decisions to individual cardiovascular risk and measured lipid outcomes.
